10% Temporary Wage Subsidy Self-Identification Form (PD27)

CRA has made this form available to be filled out for entities that are eligible for the 10% Temporary Wage subsidy to reconcile how much they have taken. It is encouraged that this is done before the end of the year to prevent receiving a notice from CRA at year end.
